• A current list of medications you are taking

• Your dental insurance information and completed forms (if applicable)

Health Conditions & Medications

Please notify us in advance if you have any medical conditions that could affect your treatment, such as diabetes, high blood pressure, heart conditions, or artificial joints. Also inform us if you are taking medications like aspirin, anticoagulants, or heart medications. This allows us to tailor your care appropriately.

Dental X-Rays

If your previous provider has taken x-rays in the past year, please have them sent to our office before your visit. If needed, you can bring them with you in person. We also offer high-quality digital x-rays on-site if additional imaging is required.
